What do I need to bring?
KCHD requires a government-issued photo identification to release medical records.
Is there a cost?
The charge for medical records is $6.50. There is no charge to send records to a provider.
How will I receive my medical record?
Medical records can be printed, placed on a CD/DVD, emailed (must be encrypted), faxed or given in person. Records that are emailed are encrypted to comply with HIPAA.
My physician is no longer in practice. How do I receive my medical records from their office?
Call the Tennessee Health Professional Board at 615-532-3202.
