What do I need to bring?

KCHD requires a government-issued photo identification to release medical records.

Is there a cost? 

The charge for medical records is $6.50. There is no charge to send records to a provider.

How will I receive my medical record? 

Medical records can be printed, emailed (must be encrypted), faxed or given in person. Records that are emailed are encrypted to comply with HIPAA.

My physician is no longer in practice. How do I receive my medical records from their office?

Call the Tennessee Health Professional Board at 615-532-3202.
