CHANT Care Coordination Services

The CHANT program provides free and voluntary care coordination services to pregnant women and any parent/guardian that has a child in the home under the age of 21, including those with special healthcare needs that qualify for our CSS program.

Developmental Screenings: For ages 5 and under. Includes education on how to improve on your child’s developmental skills and linking to the Tennessee Early Intervention System (TEIS) if further evaluation is needed.

Health Insurance: Assistance with qualifying and applying for health insurance.

Housing: Assessment of housing needs and appropriate referrals for families’ unique needs. This includes connecting families with rental assistance, referring to appropriate shelters, reviewing the Knoxville Community Development Corporation’s (KCDC) application process, connection to home repair programs and more.

Medical Referrals: Assistance with connecting to behavioral, dental and medical providers based on health insurance coverage. CHANT will also assist clients with receiving care at free or reduced cost clinics for those without insurance.

Social Services: Connect families to resources that fit their individual needs such as clothing, transportation, food, childcare and more.

Child Health and Development Education: For ages 21 and under. Includes education on positive parenting and referrals for those interested in additional parenting support.

Immunization Education and Support: For new moms and for parents who have children who are not up to date on vaccines.

Prenatal Education: Breastfeeding, safe sleep, prenatal checkups, preparing for baby's arrival and more.

Postnatal Education: Referrals for family planning, importance of postnatal checkups, awareness of postpartum depression and continued breastfeeding support.

Tobacco Cessation: Provide education and awareness about the dangers of first, second and thirdhand smoke, smoking during pregnancy and referring to resources that will aid in quitting.
