Life Insurance Open Enrollment for Knox County Employees
October 13, 2014 through November 14th, 2014
The Life Insurance Provider for 2015 has not changed, but their name has. ING’s new name is VOYA (effective October 1, 2014). Same carrier and the same rates, just a new name!
Employees of Knox County Government have an opportunity during the Open Enrollment period to increase their Supplemental Life Insurance by one step (assuming you are not already at the maximum of $105,000) without any medical questions asked. For those employees who wish to increase their Supplemental Life Insurance more than one step, a VOYA EOI Form (evidence of insurability) must be completed and returned to the Retirement Office. VOYA will notify you as to whether you are approved or denied the increase amount above one step. All elected / approved increases will be effective January 1, 2015, or later if VOYA approves the EOI after January 1. For frequently ask Q & A, (click here)
EOI Forms are available here (click here) and the instructions (click here) and are available in the Knox County Self-Service System for Open Enrollment. All Life Insurance Enrollment changes will be handled through the Employee Self-Service System.
The Rate Sheet for Supplemental Life is available here (click here).
To change / update your Life Insurance Beneficiary(ies), complete the Beneficiary Change Form (click here) (5) and return it the Retirement Office, Room 371 in the City/County Building. For additional questions, contact the Retirement Office at (865) 215-2323.
